Care package terms

Conditions of purchased packages

  1. This care plan expires after the number of months stated on your plan from the date of purchase.
  2. A patient may choose to cancel their care plan at any time while it is still valid. A refund will be given for the entire unused portion of the plan. An administrative charge of $125.00 will be charged for all cancelled packages.
  3. All care plans are per individual and cannot be shared between family members.
